Serefin Health care coordination connects individuals with a dedicated nurse care coordinator who helps them better understand and navigate healthcare and community services.
This is not emergency care. It’s relationship-based support designed to help individuals and caregivers feel more confident, informed, and connected throughout their care journey.
Nurse care coordinators help residents:

Understand their care plan

Connect with health and community services

Receive support between appointments

Prepare for referrals and upcoming visits

Access educational resources and wellness support

Get answers to health-related questions as they arise

The program is designed to support patients and caregivers who:

Have ongoing or complex health concerns

Need help navigating healthcare services

Do not have a regular family doctor

Require additional support beyond primary care

Seek guidance and coordination support

Grey Bruce Pilot Program
MyCareSteps is currently being delivered as a pilot program across Grey and Bruce counties in partnership with Brightshores Health System (referrals & evaluation) and Grey-Bruce OHT (stakeholder engagement & referrals). Serefin Health is the care coordination service provider. The Brightshores Research Institute, with support from Health Excellence Canada, is evaluating the program to understand its impact on patient experience, care coordination, and provider workflows, and to inform future models.
